Phenol Peel
The primary indication for deep phenol peels is the treatment of severe facial wrinkles. The treatments are performed alone or combined with a surgical facelift to achieve spectacular facial rejuvenation results. Since phenol peels with lower concentrations of croton oil and phenol are used, the indications have been extended to include photoaging of the skin and wrinkles of lesser severity. Other indications include facial skin discolouration of various origin, acne scars or solar keratosis.
Phenol peels cause coagulation of the epidermis and dermis to its central reticular layer. Croton oil is a very active substance whose addition to the peel increases phenol penetration. Higher concentration of croton oil results in deeper penetration of phenol.

The treatment itself involves washing the skin thoroughly and degreasing it with ethanol or acetone. Phenol peel is applied on facial skin with a gauze pad or cosmetic sticks. Immediately after application, temporary frosting is observed; however, the final effect of deep peels is the appearance of greying and then swelling of the treated area. Special attention should be paid to the area around nose wings and inner eye corners due to possible accumulation of the peel and the possibility of excessive skin damage. When treating the lower eyelids, it is necessary to keep a distance of 1-2 mm from the lash line. On the upper eyelid, the treatment should not be performed over the tarsal cartilage. In case of severe wrinkles, it is also recommended to fill the subcutaneous tissue or, in case of severe flaccidity, to combine it with surgical skin lift. Immediately after the procedure, adhesive tapes are applied to increase occlusion. Bismuth gallate is applied on the eyelids after the procedure. On the following day, the tapes are removed and bismuth gallate is applied onto the remaining treated area; this should be continued until the 7th day after the procedure. Next, the patient can apply vaseline or other regenerating preparations.

Patients must follow post-treatment instructions, accept a period of convalescence due to long healing time, refrain from regular activities for 1-1.5 weeks. Patients with a darker skin phototype are not the best candidates for the deep phenol peel due to the possibility of development of pigmentation disorders.
Deep phenol peels applied onto the whole face can be performed under general or infiltration anaesthesia. Application of intravenous or oral fluids is recommended to reduce possible toxic cardiac complications caused by phenol. Administration of sedatives before the procedure is recommended, e.g. benzodiazepines, as well as of analgesics after the procedure - NSAIDs, tramadol. In patients where only one or two cosmetic areas are treated, it is enough to administer infiltration anaesthesia and consume at least 1 litre of water or isotonic drink.
