In small children, the procedure can be performed under general (full) anaesthesia. In older children and adults, the surgery is normally carried out under local infiltration anaesthesia with possible additional sedation. Bilateral surgery to correct prominent pinnae takes about 1.5 hours. If the surgery is performed under local anaesthesia, there is no need to stay overnight at the hospital and the patient goes home after the surgery. After injecting the operated area with an anaesthetic, the plastic surgeon cuts out an elongated piece of skin from behind the ear, and then the pinna cartilage becomes The cartilage is then thinned or cut out so that it can be modelled and brought closer to the head surface. Non-absorbable sutures are placed on the cartilage to hold it in its new position. After thorough haemostasis, the plastic surgeon sutures the skin behind the ear with a continuous suture. The operation ends with a shaping and compression dressing designed to additionally hold the ears in their new position and enhance their new shape.

Recommendations after corrective ear surgery include: Limiting major physical efforts, including lifting heavy objects for the first 2 weeks. Failure to do so involves the risk of bleeding and the need for revision surgery. A sparing lifestyle ought to be pursued for at least 6 weeks. It is necessary to wear a dressing and, later, a compression band on the ears for several weeks after the operation (24 hours a day in the first period, then at night only). The dressing is changed for the first time 1-3 days after the surgery. Skin sutures are usually removed on the 10th day after the procedure. Smoking and inhaling tobacco smoke should be avoided for at least 6 weeks before surgery and throughout the healing period. Ear injuries ought to be avoided throughout the period of healing and formation of the scar, as the beneficial surgery result may be lost. The patient should refrain from using the swimming pool and sauna for 1 month after the surgery. The patient should not wear glasses, including sunglasses, for at least 1 month after the surgery. The patient should not sunbathe the skin of the operated area for 6 weeks after surgery, use high photoprotection (SPF 30-50). The patient should be aware that smoking or exposure to cigarette smoke significantly increases the risk of complications during the surgery and significantly impairs the process of proper wound healing.
