RULES FOR USING GIFT CARD (Excerpt from the Regulations)
A Gift Card is valid for a specified amount or entitles you to use the services specified on it at Klinika Ambroziak.
A Gift Cardd is personal, only the person to whom the Card was issued is entitled to use it.
Expiration date of Gift Card is one year from the date of issue, without the possibility of renewing it.

When registering, please inform call-center that the form of payment for the treatment will be the Gift Card. Please show your Card at the reception desk before using the treatment.
A Gift Card is valid only with the signature of Joanna or Marcin Ambroziak.
Settlement with a treatment card cannot be combined with promotions and discounts.
A Gift Card can be used for one or more visits depending on the services or values specified.
A Gift ard cannot be returned or exchanged for cash. It is also not possible to spend the rest on Gift Card.
A Gift Card for a specific amount can be combined with other payment methods.
The services indicated on a Gift Card cannot be exchanged for other services offered by Klinika Ambroziak.
A Gift Card can be purchased directly at the Clinic's reception desk or ordered by e-mail: [email protected].
For an additional fee, at the request of the Buyer, a Gift Card can be sent to the address indicated.
